Please provide an accurate amount accumulated in the designated line items below. This will assist our warehouse in regards to the scheduling of your pick up or to assist us in knowing how much is being dropped off.

All lamp boxes must be sealed and labeled with location of generation, spent or used lamps, quantity and date first lamp was put in the box. Boxes with Batteries, Ballasts or other items must be labeled with contents. Contact points on batteries must be taped to avoid additional fees.
